Gastric sleeve vs gastric bypass for the treatment of morbid obesity

Introduction: vertical gastrectomy, although considered a relatively new technique in bariatric surgery, has shown promise in terms of weight loss and improvements in obesity-associated comorbidities. Objective: to compare gastric sleeve and gastric bypass in the treatment of morbid obesity. Method: a systematic review was conducted on articles published in the period 2019-2024, in the EPISTEMONIKOS databases (Cochrane Library, Pubmed, EMBASE, CINAHL, LILACS, PsycINFO, DARE, The Campbell Collaboration Online Library, EPI-Centre Evidence Library, JBI Database of Systematic Reviews and Implementation) and Redalyc. Discussion: regarding weight loss, studies have shown that both methods are highly effective. RYGB tends to result in faster and more significant initial weight loss, but in the long term, the difference in weight loss between RYGB and SG is not as significant. Some argue that the more rapid weight loss of RYGB may provide greater initial motivation for patients, while others maintain that the similar long-term weight loss of SG may be more sustainable for some patients. Conclusions: both Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) are effective procedures for the treatment of morbid obesity and its comorbidities. The choice between both methods should be based on an individualized assessment of the risks and benefits, considering the patient's preferences, medical history, and the recommendations of the medical team.
