Asymptomatic chronic lymphocytic leukemia with massive splenomegaly. A case report

Introduction: Chronic lymphocytic leukemia is the most common leukemia in Western adults, with an incidence of 3-6 cases/100,000 population. Although it is usually asymptomatic, 10 % of cases are diagnosed in advanced stages with hematological complications. Objective: to describe the clinical case of asymptomatic chronic lymphocytic leukemia with massive splenomegaly. Case presentation: a 68-year-old woman with no relevant medical history was admitted with abdominal pain and massive splenomegaly. Studies revealed absolute lymphocytosis (25×10⁹/L), anemia (Hct: 0.28 L/L), and thrombocytopenia (31×10⁹/L). The bone marrow examination showed lymphoid infiltration (>90% mature lymphocytes), confirming stage IV chronic lymphocytic leukemia. Conclusions: this case highlights chronic lymphocytic leukemia as a diagnosis in unexplained splenomegaly, even without B symptoms. Early staging is crucial to optimize management, especially in elderly patients with advanced disease.
