Introduction: epilepsy is one of the most common chronic neurological disorders, affecting 1–1.5% of the world's population. More than half of cases are related to a genetic cause. Objective: to provide a holistic view of epilepsy, encompassing everything from its genetic foundations to its ramifications in the patients' daily lives. Case presentation: a 29-year-old patient was presented by his mother, who stated that her son has been experiencing complete and aggressive epileptic episodes since childhood. Case discussion: the management of refractory epilepsy is a clinical challenge that requires a comprehensive and personalized approach. Epilepsy surgery is emerging as an effective therapeutic option in selected cases. Final considerations: the management of refractory epilepsy remains a significant clinical challenge. Despite receiving multiple pharmacological treatments, these patients continue to experience seizures.
